Program objective
To strengthen the link between health services and the community through the work of trained local women, who accompany, guide and follow up people with medical and social needs, ensuring a more humane, closer and culturally competent access to health care.

Our Work

The Companions are women from the communities where we work. Partners In Health recruits, trains, supervises and provides ongoing mentoring so that they can become a trusted bridge between families and the health system. Their knowledge of the social, cultural and geographic environment allows them to provide comprehensive and close accompaniment, especially for people with chronic diseases, pregnant women, postpartum women or those in vulnerable situations.
Activities carried out in the program
Home visits:
- Activate community support networks.
- They provide basic information on health and prevention.
- They actively detect preventable conditions.
- They encourage adherence to treatment and attendance at consultations.
- They reinforce medical recommendations.
- They personalize the care and follow-up of each patient.
- They accompany pre- and postnatal care processes.
- They offer emotional support to patients and families.
Monthly meetings:
- Spaces for feedback and updating.
- Ongoing training on physical and mental health issues and social aspects.
- Delivery of information and materials necessary for their work.
- Review and data collection.
Information management:
- Filling out paper forms during visits.
Supervisors transfer data to the CommCare platform during office hours for systematic follow-up.
